By signing below, I acknowledge that I am the parent or legal guardian of the children listed on this form and grant permission for them to participate in the Girls Only Iftar program at Masjid Furqaan Bolingbrook. I understand and agree that participation is voluntary. I release and hold harmless the organizers, volunteers, staff, and hosting facility from any and all claims, liabilities, or damages arising from my child's participation. I understand and agree to the stated drop-off and pick-up times and acknowledge that I am responsible for timely pick-up of my children.
