By signing below, I acknowledge that I am participating in the Mothers & Daughters Summer Social at Masjid Furqaan Bolingbrook voluntarily. I understand that the event may include outdoor activities, games, crafts, food, and other group activities. I understand that Masjid Furqaan, Al-Furqaan Foundation, the event organizers, volunteers, staff, and hosting facility are not responsible for any injury, loss, accident, or damage that may occur during or as a result of participation in this event. I understand that I am responsible for myself and for any child attending with me, including their supervision, transportation, arrival, and pick-up.
